MEDICARE AND MEDICAID plans must be disclosed to me, as I am unable to accept payment from you. These plans pay me. Self-Pay is not available if you have insurance through these plans with no opt-out, availability. 

IN-NETWORK INSURANCE COVERAGE:

Aetna plans, Medicare Part B and some Medicare Managed plans and Optum(UHC/UMR) Plans:

-Obtain insurance verification below through O'Brien's Billing, if using insurance.

OUT OF NETWORK INSURANCE:

You will pay the fee for session before each meeting and you will receive a superbill with the information your insurance carrier will need for out of network potential reimbursement. 

If you are in-network, then your copay is due before each session.

Please know OON reimbursement is between you and your insurance carrier. I cannot guarantee they will reimburse, each plan is different, so please know this before starting services. 

My services are self-pay or OON, if not using In-Network insurance, and you can use the superbill receipt to ask your insurance provider to possibly reimburse you for your (OON)Out Of Network services. Your plan typically has an OON benefit.  Self-pay is when you do not want to use insurance for out-of-network reimbursement, or when you want privacy and do not want a mental health diagnosis in order to receive services.

Payments Include our Good Faith Agreement. You are now protected under the No Surprises Act for Mental Health Therapy Sessions when using your out of network benefits, Self-Pay, and for in-network insurance services.

You will not incur a large bill as you will be paid up to date before every session, unless there is a high or unmet deductible, and/or co-insurance due.
